Prescriptions & Refills

All prescriptions and refill requests should be requested during normal office hours. Please be sure to give us at least 48 hours notice on all refill requests.

Medical Records

A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider.

Referrals

If a referral is necessary, we request notification at least 72 hours in advance of your appointment. Same day referrals will be processed for urgent appointments./p>

Disability and FMLA forms

We are happy to complete any disability forms you will need. We charge a nominal fee for this service and request at least one week notice./p>
